This Privacy Notice applies to you as a Data Subject – Participants will be provided with a full Data Privacy Notice, Data Collection Form and Informed Consent and Waiver Form for review and completion prior to the commencement of any services.
The Lawful Basis on Which Your Data is Processed
Before your first session, I will provide you with a list explaining the legal basis for processing your data. I also will ask you to sign to agree that you are happy for me to hold your personal data, use it for the legal basis set out on the form and the purposes described in the Privacy Notice.
Who gets to see your personal data?
I only use your data for the reasons that I collected it, and I do not share your data with anyone else; unless I am required to do by law or if the safety of you or someone else is at risk.
Can you change your mind about the use of your data?
Yes, you can tell me that you no longer wish me to use your data, and if you do, I will stop using it as soon as it is reasonably practical.
Can you ask to be forgotten by me?
Yes, if you no longer wish me to have or use your data, you can tell me to delete all your data from my records, and I will do so as soon as it is reasonably practical.
What are your rights?
Under the General Data Protection Regulation, you have the following rights:
1. To be told about what data I have, how and what I use it for, and who I share it with.
2. To be given access to your personal data.
3. To have any errors corrected or incomplete data completed.
4. To have the data I hold about you removed.
5. To stop me from using your data in a way you think is unjustified or based on inaccurate data.
6. To move the data that I hold to another organisation.
